Outreach/SHINE
The Hamilton Council on Aging and Senior Center offers outreach services.
A staff member, the Coordinator of Outreach and Social Services, is available 15 hours a week to provide information and referral on local services and provide assistance on the phone, in person at the Center, or at a home visit for Hamilton residents over the age of 60.
Information provided includes:
- in-home care
- elder assessment
- assisted care and long-term care facilities
- fuel assistance
- food stamps
- transportation
- support groups
- finances
- senior scams
- health insurance
- and others
We have a certified as a SHINE counselor (Serving the Health Information Needs of Elders), who is available to assist with:
- Medicare enrollment
- plan changes during Open Enrollment
- Medicare claims
